About the Role:
As a Care Support Planner, you'll use a person-centered, strength-based approach to support carers, many of whom are navigating complex and emotionally challenging situations. You’ll be the first point of contact, guiding them through assessments, care planning, and connection to services that support their wellbeing.
Who You Are
Essential:
- Qualifications in social/community services OR significant relevant experience
- Background in intake and assessment roles
- Knowledge of local community resources and support networks
- Understanding of wellness, enablement, and consumer-directed care
- Strong communication skills and empathy for carers
- Proficient with CRM systems and general computer use
- Current Driver’s License, Working with Children Clearance (Ochre Card), National Police Clearance, First Aid Certificate
Desirable:
- 2+ years of experience working with carers or in support services
- Familiarity with Northern Territory community networks
- Skilled in person-centered and strength-based support delivery
